education course in physical therapy.Licensed in the state in which
they will be practicing.Must not be excluded or sanctioned from
state or federal program.Demonstrate professional attitude and
appearance.Proficient administrative and communication within
department and facility and company.Thorough knowledge of the
principles, methods, materials, and equipment used in the practice
of their discipline.Display working knowledge of multidisciplinary
practice scopes in the Outpatient, ALF, ILF or LTC setting.Thorough
knowledge of all payer source documentation and payment
requirements.Ability to use analytical skills to develop outcome
driven solutions.Prior EMR/computer experiencePhysical Demands:Must
be able to walk, sit, stand, stoop, squat, bend, lift, push, pull,
balance, and perform overhead activity without restriction or
limitation.Duties and Responsibilities:Must uphold the
professional, legal and ethical standards of practice for his or
her physical therapy as defined by the APTA and State Practice
ActResponsible for service and operational excellence of all
assigned activities to ensure the delivery of quality services
and/or outcomes required to meet or exceed the expectations of
those utilizing or impacted by the departmentPerforms initial
patient examination, evaluation, interpretation of referrals, and
creates the initial plan of care for each patient and revises as
appropriateCreates and maintains a patient record for each patient
treatsProvides on-site assessment and re-evaluation of each
patients treatment according to the State or Federal required
interval, whichever is more stringent, and adjusts the treatment
plan as appropriateCoordinates discharge plan and decisions with
the PTADemonstrates knowledge and appropriate application of
precautions and contraindications to physical therapy treatment
procedures and modalitiesCompletes documentation according to
Facility policies including timeliness and medical necessity
supportHas primary responsibility for physical therapy care
rendered by the PTA for all patients/clients for whom he or she is
the therapist of record. In doing so:Delegates appropriate portions
of the treatment plan and program to the PTA consistent with the
PTAs education, training and experienceMust be always accessible by
telecommunication to the PTA while the PTA is treating
patients/clients for whom he or she is the therapist of recordWhat
