United Staffing Solutions is seeking a travel nurse RN Case Management for a travel nursing job in Peoria, Illinois. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: ASAP Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Position: Case Management RN Facility: 650-bed Level 1 Trauma Center General Duties: Assess patients for discharge planning and psychosocial needs. Collaborate with interdisciplinary teams for care coordination. Manage care transitions and discharge plans for 15-20 patients daily. Required Skills: Proficient in care coordination and discharge planning. Experience in psychosocial needs assessment. Certifications: Case Management certification (preferred). Schedule: 8-hour shifts, M-F with rotating weekends. Call until 6pm daily. Floating: May float to various adult units (Med/Surg, Cardiac, Neuro, ICU). Dress Code: Business casual, comfortable walking shoes, white lab coat. Training: EPIC training, 1 day in-seat, 2-day shadowing. United Staffing Solutions Job ID 234979.
