Primary Care for a job in MADISON, Wisconsin. Job Description &

Schedule: 80% FTE, Monday – Friday between the hours of 8:00 AM –
5:00 PM. No holidays or call. Hours may vary based on the needs of
the department. Be part of something remarkable As an Advanced
Practice Provider, you will play an integral role in building and
expanding a virtual primary care program. You’ll be a part of the
ambulatory care teams by providing high-quality, cost-effective
care to patients both in person and virtually. We are seeking a
Nurse Practitioner OR Physician Assistant to: - Be a part of a
virtual care team expanding access to primary care through
telehealth visits - Care for the acute and chronic needs of
patients from newborn to elderly. - Perform physical exams and
manage the health maintenance needs of our family medicine
population. - Perform routine clinical procedures independently
including wart removal, PAP smears and skin lesion removals.
Education: - If Nurse Practitioner: Master’s or Doctorate’s degree
from accredited Nursing Program. - If Physician Assistant:
Bachelor’s or Master’s degree from accredited PA Program. Work
Experience: Required– Two (2) years of relevant clinical experience
Licenses and Certifications: Minimum – State of Wisconsin NP or PA
licensure If Nurse Practitioner*: - Active RN and APRN
certifications and licenses, including prescriptive authority, in
state(s) of practice AND - Active board certification as a NP in
area of clinical practice. These may include the following: Family.
If Physician Assistant*: - Active PA license, including
prescriptive authority, in state(s) of practice AND - Active PA
board certification All Nurse Practitioner and Physician Assistant
applicants must have*: Active DEA license or ability to apply for
such license prior to or upon hire Qualifications - Master's Degree
or Doctorate’s degree from accredited Nursing Program Required Work
Experience Licenses & Certifications - Active RN and APRN
certifications and licenses, including prescriptive authority, in
state(s) of practice Upon Hire Required and - Active board
certification as a NP in area of clinical practice. These may
include the following: Adult; Adult/Gerontology Acute Care;
Adult/Gerontology Primary Care; Emergency; Family; Gerontology;
Neonatal; Pediatric Acute Care; Pediatric Primary Care;
Psychiatric-Mental Health; or Women’s Health Upon Hire Required and
