Rates Needed for Submission:Malpractice must be included in the
ratesOrientationDaily 2800/day capHoliday DailyOT 475/hour cap to
apply after 8 hours/dayHoliday OTNight Call to include minimum of 2
gratis hours 500/night to include 2 hours gratis capHoliday Night
Call24 Hour On Call to include a minimum 4 gratis hours 2400/24
hours with a minimum of 4 hours gratis capHoliday 24 Hour On
CallCall Back 475/hour to apply after gratis hoursHoliday Call
BackConversion fee 25k rate capTravel Reimbursement:Flights -
750/round trip cap to include all taxes and fees. No upgraded
airfares or seat upgradesLodging - 1.5x the GSA rate cap average
for the total stay to include taxes and feesRental Car - 55/day cap
including all taxes and fees regardless of car class. SUV/upgrades
over cap will need to be preapproved will only be approved if
