Job Description Primary Care Physician – Chicago, IL Clinic
Locations: Bellwood, Berwyn, Brighton Park, Chinatown, Elgin,
Garfield Ridge, and South Side Compensation: $215K–$245K Base

patients’ lives. Position Highlights Compensation: Base salary
$215K–$245K (based on experience and location) Bonus:
Performance-based incentives tied to quality metrics Support Team:
Scribe, Medical Assistant, and Social Worker Clinic Resources:
Onsite lab with phlebotomist; RubiconMD specialist consultation
platform EMR Systems: Canopy and Greenway Model: 100% outpatient,
value-based healthcare Providers per Clinic: 3–5 (mix of MDs and
NPs) Patient Experience: Complimentary van transportation within 5
miles; community wellness activities including yoga, exercise,
