Job Description:

Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a committed CCS Service Facilitator to join our dedicated team in promoting health, healing, and hope in the community. This role involves coordinating comprehensive, individualized care plans for clients participating in our Community Support Programs (CSP). You will work closely with clients, their families, and treatment teams to ensure the effective delivery of mental health services and support. Assess client needs and develop individualized service plans in collaboration with treatment teams. Facilitate group sessions and support meetings for clients and families. Monitor client progress and adjust plans as necessary. Liaise with community resources to support client needs effectively. Maintain detailed records of client interactions and progress. Strong interpersonal and communication skills Ability to manage case files efficiently and responsibly Proficiency in crisis management and intervention techniques In-depth understanding of mental health challenges and community resources Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Social Worker or similar certification preferred First Aid and CPR certifications
